What Is the Pencil Test for Sagging Breasts?

For many women, breast sagging (ptosis) is a common concern that can be caused by a variety of factors such as the natural aging process, pregnancy and breastfeeding, significant weight fluctuations, and more. This droopiness can typically be addressed with breast lift (mastopexy) surgery, as it is designed to elevate the breasts to a more youthful position. In addition to the appearance of sagging breasts, many candidates for a breast lift experience skin laxity and volume loss in their bustline. 

Those unsure whether a breast lift is ideal for their degree of ptosis can perform a simple assessment. The pencil test is an easy, informal experiment you can perform at home to help determine if a breast lift may be beneficial. The steps of the pencil test include: 

  1. Remove your shirt and bra. 
  2. When standing in front of a mirror, stand up straight with your shoulders back. 
  3. Take a pencil and place it underneath your breast in the crease. 
  4. While still standing straight with your shoulders back, observe if the pencil remains in place or falls to the ground. 

If the pencil falls right away, it can mean that your level of ptosis may not be as severe and a breast lift might not be needed. If the pencil remains, that means you may be a candidate for the procedure.
